Pride Health is hiring a Home Health RN for a client in La Quinta, CA! (Contract)

Schedule: Monday-Friday 8am-4:30pm

Contract: 3 month contract

Pay Rate: 48-52/hr

Primary Responsibilities:

• Conduct home health nursing assessments to identify care, equipment, and support needs while considering participant goals and preferences.
• Reassess nursing needs regularly and update plans of care as conditions change.
• Develop and implement individualized care plans in collaboration with participants, families, and the interdisciplinary team.
• Provide and document nursing care, including medication administration, treatments, safety monitoring, and response to physician orders.
• Educate participants, families, and caregivers on medical conditions, self-care, and prevention strategies.
• Communicate changes in participant status to the interdisciplinary team and support smooth care transitions across settings.
• Identify and carry out discharge planning needs.
• Supervise and evaluate home health aide services biweekly.
• Participate in end-of-life care coordination and provide nursing coverage as needed.
• Practice within legal scope, maintain required authorization, work flexible schedules, and complete documentation accurately and on time.

Required Qualifications:

• Active RN license
• Minimum of one year of nursing experience in a clinical setting with a frail or elderly population
• Ability to initiate venous access for IV hydration or to draw labs; experienced with administering IM, SC, or transdermal injections, and providing wound care.
• Bilingual English/Spanish is a plus
